At the age of 13, this account will convert to the Minor Savings account.

You must deposit $10.00 to open this account.

An Excess Withdrawal Charge of $3.00 will be charged for each withdrawal in excess of six during a quarter.
At the age of 18, this account will convert to a Regular Savings account.

You must deposit $50.00 to open this account. A Balance Requirement Fee of $6.00 will be imposed every quarterly statement cycle if the daily balance on any day of the statement cycle falls below $200.00.

To be eligible for a Health Savings Account, you must generally: (1) be covered under a high-deductible health plan (HDHP); (2) not be covered by any other health plan that is not a HDHP; (3) not be entitled to benefits under Medicare (generally, have not reached age 65); and (4) not be claimed as a dependent on another person’s tax return.
